In most healthcare organizations, the process of "Signouts" from Hospitalist and Hospitalist or Resident to Resident is manual. The physician uses paper to summarize their daily notes for a patient. When they are finished with their rotation, they either hand over the paper notes, leave voice messages or post their findings in a physician bullpen for the next physician assuming responsibility for the patient.
This manual process impacts patient safety and confidentiality, as well as physician productivity, compliance and satisfaction.
These factors have lead the Joint Commission to institute a new requirement in the National Patient Safety Goal, "To Improve the Effectiveness of Communication Among Caregivers" by implementing a "standardized approach to "hand-off" communications".
